Pathologic 3 is a psychological horror game in which you play Daniil Dankovsky, a rationalist doctor trying to defeat death itself as the same doomed town collapses around him. It is a standalone follow-up to Pathologic 2 and a reimagining of the Bachelor route from the original Pathologic, rather than a direct sequel or prequel. Its major new distinction is time manipulation: the premise remains a 12-day plague crisis, but the Bachelor can revisit and alter key events, reflecting his obsession with control, causality, and conquering death.

Released 9 January 2026 for Xbox Series X|S, PlayStation 5 and PC (Microsoft Windows) by Ice-Pick Lodge, published by HypeTrain Digital. Rated M by ESRB, 18 by PEGI. About 32.5 hours to beat, 41 hours for everything.

Story

Bachelor Daniil Dankovsky is a young doctor from the capital. His search for the secret of immortality brings him to a remote town, gripped by a mysterious plague that will destroy it in 12 days. Now it’s up to him to explore the town itself, its past, present, and future — to find answers to his numerous questions and try to stop the relentless epidemic.
