Anathema is a reasonably difficult, vanilla compatible episode for Heretic which uses HHE/Hehacked to make several gameplay changes. Since it relies on vanilla specific formats, it is not compatible with advanced ports such as UzDoom, GzDoom, ZDoom, the Heretic+Hexen Re-release or Dsda-Doom.
